Most of all, we believe in dignity and respect for each patient we serve.nAs an Admissions Coordinator, you will:nContact the patient and family within 1 hour of referral, and coordinating all admissions within the 3-hour admission Commitment.nProcess referrals by obtaining sufficient admission and insurance information in order to ensure appropriate and timely admission of patients.nMaintain communication with pending patients, their families, and referral sources and routinely follow the progress of prospective patients and those referrals not taken under care (NTUC).

nParticipate in morning Stand-Up meetings.nParticipate as a member of the marketing team by: actively seeking new referral sources; developing and maintaining positive relationships with referral sources; assist in developing strategies and plans for Site and Company growth; participating in marketing meetings, staff meetings, department meetings, staff meetings, in-services, committees, community-education events and other activities as needed.nPrepare marketing reports regarding referral statistics.

Ensure high level of integrity of referral data within Company computer system.nMaintain working knowledge of, and assist in, accessing community resources.nParticipate in evening/weekend on-call or overtime as required.nPerform admissions presentations in the field and via phone, as directed by ED, to assist admission team in meeting 3-hour admission standard.nEnsure maximum reimbursement through initial payer source, benefits, and eligibility verification, authorization processes, and accurate data entry into Company computer system.

Completes required payer source verification forms.nQualificationsnStrong customer service experiencednHospice or medical office experience highly preferrednLPN/LVN certification or Bachelors degree in business, social work, psychology or nursing or equivalent business experience preferrednAble to work in an interdisciplinary settingnAbility to maintain business and clinical files in a systematic and orderly fashionnMust have strong communication, organization, negotiation and public relation skillsnProficient with the PC including the MS suite of products
